Hospice of New Jersey is looking for a self-motivated and passionate Hospice RN Clinical Liaison to join our Team. We offer the opportunity to take on new challenges and a culture of teamwork that sets us apart.

The RN Clinical Liaison is primarily responsible for hospital and medical facility-based outreach to identify new referral sources and maintain ongoing relationships that will increase census and preference for hospice services. Registered Nurse Liaisons use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services. Our Admission Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a Patient's home or in a health care facility. This position works with the intake team, referring physician, and other referral sources to promote the hospice philosophy of care.

The Hospice Program Director will provide overall leadership for their hospice agency operations, with a focus on growth, external market relationships, financial management, customer service and quality care delivery.
As the Hospice Administrator, you will take on a dynamic role in overseeing and administering all aspects of our day-to-day operations. You will work closely with established goals, objectives, policies, and regulations of our esteemed company. Your responsibilities will include ensuring compliance with all hospice-specific regulations, laws, and policies, including those related to Medicare/Medicaid matters. Under the guidance of senior management, you will expertly manage the affairs of our local office, making a significant impact on the lives of patients and their families during difficult times.
